Summary

During cleaning of a tank which was previously in heavy fuel oil service, breathing apparatus (B/A) was in use. A coupling on the B/A air supply hose became unscrewed, whilst the contractor using it was inside the tank. The loss of air was immediately recognised, and the contractor switched to his emergency back-up cylinder and exited the tank safely.

In subsequent investigation it was discovered that this issue had arisen previously, and that the ‘TUG TEST’, which is required as part of pre-use checks, had been undertaken on both occasions. It is therefore recommended that a further “twist test” is carried out prior to use of such lines.
