Summary

Truck carrying anhydrous ammonia began leaking chemicals on the highway. The highway was closed and a 1 km exclusion zone was put in place when the truck began leaking

Proximate causes:
• Inadequate assessment of potential failure (reports suggest a pressure relief valve on the truck had failed)
• Inadequate work planning
